What Spravato is

Spravato (esketamine) is a prescription nasal spray for depression. It works differently from most antidepressants, and it can only be given at certified treatment centers. You take each dose yourself in our office while our clinical staff stays with you and monitors how you feel.

Who may be eligible

Spravato is FDA approved for adults 18 and older who have:

  • Treatment-resistant depression, meaning depression that has not improved enough after trying at least two antidepressants

  • Major depressive disorder with acute suicidal thoughts or actions, used together with an oral antidepressant

It may not be safe for people with certain blood vessel conditions such as an aneurysm, a history of bleeding in the brain, or blood pressure that is not well controlled. Your provider will review your full medical history and current medications before recommending treatment.

Insurance and cost

Many commercial insurance plans and Medicare cover Spravato, usually with prior authorization. Before your first treatment, our team checks your benefits, submits the prior authorization for you, and explains what you can expect to pay.

If you have commercial insurance, the Spravato withMe program from the manufacturer may help lower your out-of-pocket cost. We can help you enroll.

What to expect and timing

  • Consultation: a short call with our team to answer your questions and review whether Spravato may be a fit.

  • Evaluation: a visit with one of our psychiatric providers to confirm Spravato is right for you.

  • Insurance approval: prior authorization often takes a few weeks, depending on your plan. We keep you updated along the way.

  • Treatment sessions: plan for about two hours per visit. After each dose you stay with us for at least two hours while we monitor you.

  • Schedule: twice a week for the first four weeks, once a week for weeks five through eight, then weekly or every other week based on how you respond.

  • Getting home: you cannot drive until the day after treatment, so please arrange a ride home after every session.
